Employee 1 was using a crane truck to pull pipe out of a wet well to be cut for removal. As the pipe was brought out of the 40-foot well, employee 2 rolled the pipe to be cut on the other side. While it was still connected to the hoisting cable, the pipe kicked back and knocked employee 1 into the wet well. Employee 1 sustained a right wrist fracture, back fracture, and a torn aorta. | |||||

An employee was unloading steel parts from a storage bin, which was about 4 x 8 x 3 feet. During the unloading process, the weight in the bin shifted and the edge lowered onto the employee's right boot, smashing his right pinky toe and resulting in a partial amputation. He was wearing steel-toe boots at the time. |
